We are happy to register any patient living within our practice area. If you expect to be here for only a short time (less than six months), we can also provide temporary registration.
In order to register as a patient, we require one proof of address, e.g. a current utility bill, council tax book, tenant agreement (either private or council tenant agreement), driving license, or passport.
We will not register children on their own. Ideally, we prefer to register whole families rather than only individual members. When this is not possible, we will only register a child if he/she is registering with his/her legal guardian, parent or whoever has legal responsability for the welfare of this child.
You can obtain all the required registration forms from the reception desk. Please, take your time to complete them. Once completed, please bring them back to reception, alongside identification document and proof of address. The receptionist will do the rest.

We have just expanded our practice area. The area is now bordered by Lewisham Park/Lewisham Hospital to the north, Doggett Road to the west, Hither Green Lane/Torridon Road to the east, and Bellingham Road to the south.
